Established in 1978, we offer a full continuum of care, including hospice, palliative care, home health, grief and loss counseling, and our signature PACE (Program of All-Inclusive Care for the Elderly) program. Our mission centers on "meeting individual needs through innovative care solutions," and we serve people across multiple counties in northeast Kansas. Home Health Program Provides short-term, skilled medical care to individuals in their homes to support recovery, manage chronic conditions, and help patients remain as independent as possible. Services are delivered by a coordinated team of healthcare professionals and may include nursing care, therapy, and education to help patients and families manage health needs safely at home. The program works closely with physicians and caregivers to create personalized care plans that promote healing, prevent hospital readmissions, and improve overall quality of life. Occupational Therapist Evaluates patients in their homes to assess their physical, cognitive, and functional abilities and develops personalized treatment plans to help them safely perform daily activities. They provide therapeutic interventions to improve strength, coordination, balance, fine motor skills, and mobility while teaching patients strategies for completing tasks such as bathing, dressing, grooming, meal preparation, and other essential activities of daily living. The Occupational Therapist also recommends adaptive equipment and home modifications to enhance safety and independence, educates family members and caregivers on proper support techniques, and monitors patient progress by adjusting care plans as needed. Working closely with physicians, nurses, therapists, social workers, and other members of the interdisciplinary team, they coordinate comprehensive care, participate in care conferences, and maintain accurate documentation in compliance with Medicare and regulatory standards. *****Position Start Date 1/1/2027****
  • QualificationsRequired Graduate of an accredited Occupational Therapy program Current Occupational Therapist license in the state of Kansas Current CPR/BLS certification Valid driver's license, reliable transportation, and automobile insurance Ability to travel throughout the Topeka area Strong clinical assessment, documentation, and communication skills Knowledge of Medicare home health regulations and documentation standards (or willingness to learn) Preferred One or more years of occupational therapy experience, preferably in home health, rehabilitation, acute care, or skilled nursing Experience with electronic medical records (EMR) Familiarity with OASIS documentation and Medicare home health requirements Experience treating geriatric patients and individuals with neurological, orthopedic, or chronic medical conditions Excellent time management and ability to work independently What's in it for you? Requirements
