Atlantic Visiting Nurse is seeking a dedicated Hospice On-Call Registered Nurse to provide after-hours clinical support to hospice patients throughout our community. In this rewarding role, you'll respond to urgent patient needs, provide expert symptom management, support families through end-of-life care, complete hospice admissions, and collaborate closely with our interdisciplinary team to ensure seamless, compassionate care around the clock.

PM and 8:00 AM in accordance with agency scheduling.
Respond promptly to after-hours triage requests and provide skilled nursing visits as needed.
Manage urgent patient needs, including pain and symptom management, emotional support, and crisis intervention.
Respond to patient deaths and perform hospice pronouncements with compassion and professionalism.
Clinical Assessment & Patient Care
Perform comprehensive nursing assessments and evaluate changes in patient condition.
Deliver expert pain and symptom management using evidence-based hospice practices.
Conduct hospice admissions as needed.
Develop and modify patient interventions based on changing clinical needs.
Reinforce the established Plan of Care through skilled nursing interventions and patient education.
Care Coordination & Collaboration
Collaborate with attending physicians, Hospice Medical Directors, and the interdisciplinary team to coordinate care.
Assist Hospice Case Managers by completing scheduled after-hours visits.
Coordinate durable medical equipment (DME), medications, supplies, and community resources after business hours.
Communicate patient updates to the interdisciplinary team to ensure continuity of care.
Collaborate with hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers to support seamless transitions.
Documentation & Quality
Complete accurate, timely EMR documentation following each patient encounter.
Maintain regulatory compliance with Medicare Hospice Conditions of Participation and agency standards.
Report safety concerns, incidents, and changes in patient condition.
Review patient documentation and care plans to prepare for scheduled visits.
Provide detailed shift reports to ensure continuity of care.
Leadership & Professional Practice
Supervise evening Home Health Aides.
Participate in staff meetings, education programs, and mandatory training.
Maintain professional licensure and continuing education requirements.
