About the role

The Nurse Practitioner will call members to discuss open care gaps, place necessary orders, and manage patient records within Athena. They are also responsible for gathering medical histories and coordinating social needs to facilitate gap closure.

The Nurse Practitioner (VSP) works on problems of diverse scope and complexity ranging from moderate to substantial.

This is a variable service provider NP/PA (APP) role with an average of 20-30 hours per week.

Licensure preferred in TX, AZ, VA, FL, MS.

This is a fully remote position.

Responsibilities

Call Humana members to discuss open care gaps and placing needed orders (specifically women who had a fracture in the last 6 months) Create and register a patient record within Athena You will verify benefits You will gather medication, medical and surgical history Coordinate management of members social needs to help facilitate gap closure – providing transportation services.

Travel

While this is a remote position, occasional travel to Humana's offices for training or meetings may be required. Scheduled Weekly Hours 1 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

Requirements

Requires a Master of Science in Nursing and an active, unrestricted NP license, preferably in TX, AZ, VA, FL, or MS. Candidates must have at least 2 years of experience and meet specific home internet speed requirements.
