Licensed Vocational Nurse (LVN) Concord Post Acute is seeking a compassionate, dependable, and skilled Part-Time Licensed Vocational Nurse (LVN) to join our dedicated nursing team. The LVN will provide high-quality direct patient care, support clinical staff, and help ensure positive outcomes for our residents in a fast-paced post-acute care environment. This position is ideal for an individual who is committed to delivering exceptional, resident-centered care while working collaboratively with an interdisciplinary team. Position Responsibilities Provide direct nursing care in accordance with established care plans and nursing standards Administer medications and treatments as prescribed by physicians Monitor, assess, and document resident conditions, reporting changes promptly to the Registered Nurse or physician Assist with resident admissions, discharges, and transfers Collaborate with physicians, nurses, therapists, and other healthcare professionals to ensure continuity of care Maintain accurate and timely clinical documentation in compliance with state and federal regulations Promote a safe, clean, and supportive environment for residents, visitors, and staff Deliver compassionate, respectful care while maintaining resident dignity and independence
