Weekend Baylor Registered Nurse (RN) – Home Health Make a real impact with a schedule that works for you. Our Weekend Baylor RNs provide one-on-one skilled nursing care in the home, helping patients maintain independence while receiving high-quality, personalized care. This role offers autonomy, strong team support, and meaningful patient relationships. Schedule (Full-Time, Exempt) Saturday & Sunday: 7:00 AM – 8:00 PM Monday: 4 hours for follow-ups and documentation Key Responsibilities Perform patient assessments and develop individualized care plans Deliver skilled nursing care, including medication and treatment management Monitor patient progress and adjust care as needed Educate patients and caregivers on health management and self-care Coordinate with physicians and interdisciplinary teams Supervise Home Health Aides and LPNs Complete timely and accurate clinical documentation (including OASIS)
Active RN license BSN + 1 year experience OR ASN/Diploma + 2 years experience Current CPR certification Valid driver’s license and reliable transportation Preferred Home health experience Behavioral or mental health experience Physical Demands & Work Environment Frequent standing, walking, sitting, and use of hands Regular bending, kneeling, and reaching Occasional lifting and repositioning of patients Ability to lift/push up to and over 25 lbs as needed Exposure to clinical environments and infectious materials Join a team where you can deliver meaningful, patient-centered care—right where it matters most.
