Job Overview We are seeking a dynamic and compassionate Home Health Physical Therapist to join our dedicated healthcare team. In this vital role, you will deliver personalized physiotherapy services directly to patients in their homes, promoting recovery, independence, and improved quality of life. Your expertise will help bridge the gap between hospital care and community living, ensuring seamless care transitions and comprehensive treatment plans. This position offers an exciting opportunity to make a meaningful difference in patients’ lives while working in a flexible, supportive environment.

Responsibilities

  • Conduct thorough patient assessments to develop individualized care plans tailored to each patient’s needs, including those in pediatrics, geriatrics, orthopedics, and rehabilitation settings.
  • Deliver expert physiotherapy interventions such as trigger point therapy, mobility training, strength exercises, and pain management techniques.
  • Coordinate discharge planning with healthcare teams to ensure smooth transitions from inpatient or outpatient settings to home care.
  • Educate patients and their families on injury prevention, proper exercise techniques, and condition management to foster long-term health benefits.
  • Utilize electronic medical record (EMR) systems efficiently to document patient progress, treatment plans, and care updates accurately.
  • Collaborate with multidisciplinary teams across acute care hospitals, outpatient clinics, and community health programs to optimize patient outcomes.
  • Monitor patient progress regularly and adjust care plans accordingly to maximize recovery potential and functional independence.
  • Experience
  • Proven experience in home health or community-based physiotherapy is highly preferred.
  • Strong knowledge of physiology, anatomy, and medical terminology essential for accurate assessment and treatment planning.
  • Hands-on experience with inpatient hospital care, outpatient clinics, or outpatient therapy environments is advantageous.
  • Familiarity with discharge planning processes and managing care for diverse populations including pediatrics and geriatrics.
  • Proficiency in using EMR systems for documentation and communication within healthcare teams.
  • Background in acute care settings or hospital experience that provides a solid foundation for managing complex cases.
  • Knowledge of orthopedics, trigger point therapy, and physiotherapy techniques tailored for home & community care environments.
