About the role

The Case Manager will provide comprehensive case management and housing support services to individuals aged 55 and older with chronic health conditions. They will assist residents with daily living activities and provide training to help them successfully maintain their on-site housing.

RiseWell Community Services is seeking a full-time Case Manager to join our Affordable Residential Housing Team in Brooklyn, NY

In this role, you will provide case management services, housing support and assistance to individual’s age 55+, who have chronic health conditions and need assistance with activities of daily living. Assist residents with overcoming barriers associated with their health conditions so they may live in on-site in their apartments successfully. Provide information, training, hands on assistance and skill building to individuals served.

Qualifications

BA in Human Services or equivalent. Experience working with people with disabilities or older adults a plus. Clean, valid NY State Driver’s License. Fingerprinting, criminal record check, approval from NYS Office of Mental Health.

Requirements

Candidates must hold a Bachelor's degree in Human Services or an equivalent field and possess a clean, valid NY State Driver's License. The role requires successful completion of fingerprinting, a criminal record check, and approval from the NYS Office of Mental Health.
