PURPOSE STATEMENT: Plan, direct, define and coordinate individualized and group therapeutic support and services to patient’s with mental health, behavioral health and substance abuse issues in an outatient setting.
Provides a broad range of therapeutic and psychiatric social work services to mentally or emotionally disturbed patients. Acts as a liaison with hospitals, clinics, referral sources, and other medical professionals/centers affiliated with the facility. Collaborates with psychiatric and interdisciplinary teams in diagnoses and treatment plans. Assess relative, cognitive, behavioral or emotional problems. Other Duties as assigned OTHER FUNCTIONS: Perform other functions and tasks as assigned.
Qualifications or Skill Required: Master's degree in human services field. Must be licensed in the State of Illiniois; limited or full licensure welcomed.
Candidates must hold a Master's degree in a human services field and possess a current Illinois license as an LMSW or LPC. One to three years of experience in community mental health or a related setting is preferred.
