As a PMHNP with MindCare Solutions, you will collaborate with interdisciplinary teams to deliver high-quality, value-based psychiatric care across a variety of clinical settings. You will focus on proactive, evidence-based treatment strategies that improve the health and well-being of individuals while strengthening the capacity of partner organizations. Conduct Comprehensive Psychiatric Evaluations: Perform thorough psychiatric assessments to establish a baseline, manage psychotropic medications, and deliver personalized, need-based care. Ongoing Medication Management: Develop and adjust treatment plans, including medication management, based on patient progress and input from the interdisciplinary team. Collaborative Care Planning: Partner with case managers and interdisciplinary teams to create and refine care plans, ensuring a seamless, integrated approach to behavioral health management. Team Engagement: Participate in regular team meetings to evaluate new referrals, review active cases, and adjust care plans based on patient progress and organizational goals. Engagement in Quality Programs: Contribute to quality initiatives such as performance improvement programs, medication reviews, and outcome optimization strategies.
Travel up to 50% of the time to meet with healthcare partners, attend industry events, and provide consistent support across clinical settings.
Master’s or Doctoral degree in Nursing from an accredited program specializing in Psychiatric Mental Health Current and unrestricted PMHNP license in the state of MO Board certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC). Must have prescriptive authority and DEA. Clinical Expertise: Proficiency in psychiatric evaluations, psychotropic medication management, and evidence-based treatment planning. Interpersonal and Team Collaboration Skills: Strong ability to build relationships and work collaboratively with interdisciplinary teams in diverse care settings. Community-Focused: Awareness of social determinants of health and access to resources to address holistic patient needs. Tech-Savvy: Comfortable using electronic health records (EHRs), telehealth platforms, and other digital tools to enhance care delivery.
