About the role

Provide evidence-based individual, family, and group therapy while managing community-based case management for clients. Collaborate with a multidisciplinary team and maintain accurate clinical documentation in compliance with regulatory standards.

What you'll do

Provide individual, family, and group therapy using evidence-based, recovery-oriented practices.

Complete behavioral health assessments and develop individualized treatment plans.

Deliver community-based case management and connect clients to community resources.

Respond to crises and advocate for client needs.

Collaborate with psychiatrists, nurses, peer support specialists, and community partners.

Maintain accurate clinical documentation and ensure compliance with agency and regulatory standards.

Participate in the FACT team's rotating on-call schedule.

Qualifications

  • Required
  • Bachelor's degree in Social Work, Counseling, or a related field
  • Ohio LSW or LPC license
  • Valid Ohio driver's license
  • Preferred
  • LISW or LPCC
  • Experience working with individuals with severe and persistent mental illness
  • Community-based behavioral health experience
