The stated pay scale reflects the range that Sharp reasonably expects to pay for this position. The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do Community Health Workers will primarily be working out in the community with specific target populations. CHW's will work closely with medical providers, primary care teams, and other agencies to improve patient care and outcomes.

  • Required Qualifications 2 Years combined health/social services experience and/or education. Driver's License - CA Department of Motor Vehicles

Other Qualification Requirements Utilizes reliable transportation and possesses adequate personal insurance coverage. Demonstrates clean driving record in accordance with requirements of the employer DMV pull notice program and Sharp HealthCare Driver Guidelines.

Essential Functions Act as a resource or representative Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.

Assist clients in accessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of the medical home, overcoming barriers to obtaining needed medical care and social services.

Follow-up with health management/care plans with both patients and providers.

Program education Convey the purposes and services of a program to the user population and the impact that program or service would have.

Record management Document activities, service plans, and results in an effective manner while strictly adhering to the policies and procedures in place.

Work collaboratively and effectively within a team.
