Who We're Looking For

The Case Manager ensures a safe and supportive environment for patients throughout treatment. This role assists with orientation, admissions, discharge planning, and documentation, while serving as a liaison between patients, the clinical team, and admissions staff.

What You'll Do

Conduct comprehensive assessments to identify clients' needs and develop individualized care plans.

Coordinate and facilitate access to various services, including therapy, medical care, and community resources.

Monitor clients' progress and adjust care plans as needed, ensuring effective treatment and support.

Provide ongoing support and counseling to clients, helping them navigate challenges and maintain motivation.

Collaborate with healthcare professionals, family members, and community organizations to ensure a cohesive support system.

  • What You Need
  • Knowledge of addiction and recovery preferred
  • Knowledge of CARF, DCF and Joint Commission Standards, preferred
  • Strong interpersonal and communication skills for working with patients, families, and interdisciplinary teams.
  • Knowledge of mental health conditions, treatment modalities and community resources.
  • Bachelor’s degree in social work, psychology, counseling or related human services field preferred.
  • Prior experience in behavioral health, mental health, or substance use treatment setting preferred.
  • Current CPR Certification
  • Crisis Prevention Intervention (CPI) Training
  • Successful clearance of a Level II background check

Requirements

Candidates should have a bachelor's degree in a human services field and prior experience in behavioral health or substance use treatment. Essential certifications include current CPR and Crisis Prevention Intervention (CPI) training, along with the ability to pass a Level II background check.
