Responsibilities

  • Assess and treat patients requiring physical therapy services within a skilled nursing setting
  • Develop, implement, and monitor individualized therapy plans, including gait training, strengthening, and functional mobility
  • Perform initial evaluations, document progress, and adjust treatment plans based on patient response and physician orders
  • Collaborate with an interdisciplinary team to optimize patient outcomes and discharge planning
  • Maintain accurate, timely documentation and ensure compliance with facility policies and regulatory requirements
  • Promote patient safety, infection prevention, and appropriate equipment use during therapy sessions

Requirements

  • Active Physical Therapist license in Pennsylvania
  • Doctor of Physical Therapy (DPT) degree or equivalent
  • Ability to work in a skilled nursing / long-term care environment
  • Strong communication and teamwork skills with the interdisciplinary care team
  • Availability to work the guaranteed 35-hour weekly schedule on the 8:00 AM–4:30 PM shift
