The GUIDE Care Navigator will provide comprehensive care coordination, medication reviews, and disease management education for Medicare beneficiaries living with dementia. This role involves conducting home visits, managing outreach and enrollment, and collaborating with an interdisciplinary team to ensure holistic support.
As a JASA GUIDE Care Navigator, you will play a critical role in delivering Montefiore’s Guiding an Improved Dementia Experience (GUIDE) program in the community. Working with traditional Medicare beneficiaries living with dementia and their caregivers, you will provide comprehensive care navigation, intensive disease management education, medication reviews, and access to respite care to help seniors maintain their quality of life in the community.

Skills: Proficiency with Electronic Health Record (EHR) systems and standard computer applications; exceptional documentation and organizational skills.
