The Community Health Worker provides personalized care, education, and case management to clients to address perinatal and community-based disparities. They also facilitate workshops, manage client referrals, and maintain accurate documentation of services and outcomes.

Description Summary

The Community Health Worker is responsible for addressing perinatal and community-based disparities and assisting in improving those factors by providing personalized care and support to the clients. They act as a liaison for the client by providing community resources, education, and skill building sessions to promote wellness and self-sufficiency. The position requires the employee to maintain proper case management, client follow-up, and outsourcing for additional resources to meet clients’ needs.

Requirements

Position Responsibilities: *Provide information on perinatal support, mental health resources, nutrition and self-sufficiency. *Conduct workshops, classes, and group sessions to educate the community. *Offer one-on-one education and support to individuals and families in-office, virtually, or via in-home visits. *Identify clients specific needs/concerns and provide appropriate referrals for community and social services. *Provide beginning to end case management to assigned clients on caseload. Schedule appointments and provide reminders for follow-up visits. Organize and participate in community events and health fairs. Distribute educational materials and promote health awareness. Data entry within 48 hours of service or second business day based on date of appointment Prepare reports and document outcomes to evaluate program effectiveness. Collaborates with other team members to monitor and track clients who may need a screening or assessment Assist with processing and tracking of client referrals via email, referral screening calls, and online forms Keep current and evidenced based knowledge by attending continuing education opportunities Assist individuals in accessing resources specific to client and/or family needs. Check email and work phone daily. Attend, participate, and/or facilitate in staff meetings and other training offered by the program. Assist with coordination and/or facilitation of classes and other workshops, as needed Engage in community engagement activities, such as resource fairs and presentations. Must complete all required professional development assigned in a timely manner. Must be sensitive to the service population’s cultural and socioeconomic characteristics. Adhering to safety training and protocols on a daily basis, and taking precautionary measures to ensure the safety and well-being of self and others. Responsible for protecting the confidentiality of any information or material obtained in the service with the organization to include but not limited to client names and information, services rendered to clients, donors names and gifts, internal and external investigations or results of any investigations, and financial information. Adhere to the Code of Conduct of the agency and the code of ethics and standards of doulas. Every employee is required to take a solution-oriented approach in their interactions and undertakings, and work as a team member that promotes collaboration and commitment to the Mission and Vision of the organization. Other duties as assigned by SABD Leadership to fulfill the goals and mission of San Antonio Birth Doulas and the needs of Catholic Charities.

Minimum Qualifications

  • Education High School Diploma or equivalent Experience Community Health Worker certificate or at least two years of work with a health care provider or community-based organization.
  • Caseworker or medical assistant preferred.
  • Familiarity with concepts and philosophies such as informed consent, evidence-based practice, trauma-informed practice.
  • License and Credentials Must be at least 21 years of age Reliable transportation Valid driver license and clean driving record Valid registration Valid vehicle insurance Minimum Knowledge, Skills, and Abilities: Must pass annual background checks Excellent interpersonal skills Knowledge and ability to navigate Zoom, Microsoft Office 2010 or more recent productivity suite, including MS Word, MS Excel, and MS Outlook, MS Forms, TEAMS, SharePoint to include touch screen and “smart” devices Demonstrate knowledge of, and sensitivity to, the needs and interests of clients and their families Demonstrate organizational skills and ability to multitask Ability to maintain appropriate ethical boundaries and professional relationships with clients, doulas, and staff Understanding of the unique position and needs of a community-based doula program Bilingual preferred (English/Spanish) Extensive working knowledge of pregnancy and childbirth; A solid grasp of evidence-based practices and informed consent; Must be detail oriented, organized, self-motivated, work well independently and on a team; Must have good written and verbal skills; Must have good critical thinking and problem-solving skills.
  • Travel Requirements: Travel requirements estimated for the position includes 90% local and 10% outside of San Antonio/Bexar County, to remain within the 19 counties of CCAOSA.
