About the role

The hospice scheduler coordinates patient visits and care team assignments to ensure efficient service delivery. They also serve as a primary point of contact for patients and families to manage scheduling conflicts and facilitate communication.

Job Summary

As a Hospice Scheduler, you will play a vital role in coordinating and managing the scheduling of hospice services for our patients. Your primary responsibility will be to ensure the timely and efficient coordination of patient visits and care team assignments. Additionally, you will serve as a point of contact for patients, families, and caregivers to address scheduling concerns and facilitate communication between all parties involved in the care process.

Job Description

The Work: Coordinating and scheduling patient visits, including admissions, routine visits, and specialized services, in accordance with patient needs and care plans. Assigning appropriate care team members to patient cases based on availability, expertise, and patient preferences. Communicating with patients, families, caregivers, and healthcare providers to coordinate schedules, address scheduling conflicts, and provide updates on visit schedules. Ensuring accurate and complete documentation of scheduling information in electronic medical records and scheduling systems. Collaborating with interdisciplinary team members, including nurses, social workers, chaplains, and volunteers, to ensure coordinated and comprehensive care for patients and families. Participating in team meetings, case conferences, and other collaborative efforts to optimize patient care delivery and outcomes.

Qualifications

Prior experience in healthcare scheduling, preferably in a hospice or palliative care setting. Understanding of hospice care principles, practices, and regulations. Excellent communication and interpersonal skills, with the ability to effectively communicate with patients, families, caregivers, and healthcare providers. Detail-oriented with strong organizational skills and the ability to prioritize tasks in a fast-paced environment. Proficiency in using electronic medical records and scheduling systems. Compassionate and empathetic attitude towards patients and their families. Ability to work collaboratively as part of a multidisciplinary team to achieve common goals. Compensation Information: $20.20 - $35.35

The hourly pay range shown represents the base pay range for this position. Individual pay is determined based on factors such as experience, qualifications, skills, and market considerations. In addition to base pay, eligible team members may earn additional compensation through shift, weekend, holiday, on-call, and other applicable differentials.
