Complete an initial assessment of the patient and family to determine hospice needs Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Provides coordination amongst the clinical team supporting the patient, and supervising LPNs and Hospice aids who may be involved in supporting patients. Spend quality time with the patient to assist with pain management and support Modify and re-evaluate the plan of care when necessary Monitor, record, and report symptoms and changes in the patient's condition Obtain and receive physician orders as required Participates in on-call duties as defined by the on-call policy Other duties as assigned
of Hospice Night Nurse: Must be a Registered Nurse with a current license to practice nursing in the Commonwealth of Virginia At least one year of hospice experience is required. Graduate of an accredited nursing school. Must have a valid driver's license and a reliable car available for use during work hours.
