What you'll do

As a Case Manager, you will

Participate as a member of a multidisciplinary team providing holistic, recovery-oriented services.

Assist in developing and reviewing recovery plans in collaboration with participants, clinical staff, medical providers, and other stakeholders.

Develop and maintain relationships with community agencies and service providers.

Connect program participants with critical community-based resources and supports.

Assist individuals in achieving recovery plan goals through advocacy, education, and service coordination.

Work directly in the community within the program's service area.

Transport program participants to appointments and community resources as needed.

Complete assessments, documentation, and reports in accordance with organizational, funding, licensing, and accreditation requirements.

Perform additional duties as assigned.

Qualifications

  • Required
  • Two years of experience providing support to individuals involved in behavioral health, addiction recovery, intellectual and developmental disability services, homelessness services, or related human services programs.
  • Strong interpersonal and communication skills.
  • Intermediate computer skills.
  • Ability to work independently in community-based settings.
  • Ability to complete documentation accurately and timely.
  • Subject to applicable background clearances and employment screenings.
  • Preferred
  • Bachelor's Degree in Human Services or a related field.
  • One year of Case Management experience.
  • Experience working within behavioral health or community support programs.

  • Local travel throughout the service area required
