Participate as a member of a multidisciplinary team providing holistic, recovery-oriented services.
Assist in developing and reviewing recovery plans in collaboration with participants, clinical staff, medical providers, and other stakeholders.
Develop and maintain relationships with community agencies and service providers.
Connect program participants with critical community-based resources and supports.
Assist individuals in achieving recovery plan goals through advocacy, education, and service coordination.
Work directly in the community within the program's service area.
Transport program participants to appointments and community resources as needed.
Complete assessments, documentation, and reports in accordance with organizational, funding, licensing, and accreditation requirements.
Perform additional duties as assigned.
