- Range: $35.50 - $53.25 Atrium Health Transitional Care Clinic- For 30 days following your release from the hospital, our dedicated team of doctors, pharmacists and social workers will provide the care and support you need. Our team will work with you to provide the resources, tools and education to help you have the safest, quickest and healthiest recovery possible. We can provide transitional care in our office, virtually or at your home, depending on your needs and available resources.

- You’ll Do: As a skilled and compassionate RN, you will play a pivotal role in providing and coordinating comprehensive patient care through the nursing process to deliver safe, therapeutic care in accordance with established standards, policies, and procedures. Using evidence-based practice, clinical decision making, compassion, and skills communication while leading efforts to create the safest patient environment and the best patient experience across the continuum.
