Key Responsibilities: Conduct comprehensive mental health assessments, including clinical interviews, psychosocial histories, and mental status examinations. Develop and implement individualized treatment plans based on patient needs, diagnoses, and goals. Provide individual, family, and/or group therapy using evidence-based modalities appropriate for the patient population. Diagnose mental health disorders according to DSM-5 criteria and coordinate care with psychiatrists, nurse practitioners, primary care physicians, and other providers within the multi-specialty practice. Educate patients and families about mental health conditions, coping strategies, treatment options, and community resources. Maintain accurate, thorough, and timely clinical documentation in electronic health records in compliance with regulatory and ethical standards. Monitor patient progress, adjust treatment plans as needed, and provide crisis intervention when appropriate. Participate in case consultations, team meetings, and quality improvement initiatives. Stay current with best practices, continuing education requirements, and licensure renewal obligations. Education: Master's degree in Mental Health Counseling, Psychology, Social Work, or a related field from an accredited program.
Must have a New York State license as a Licensed Mental Health Counselor (LMHC).
