Role Overview

The LPN is responsible for supervising day-to-day health services activities. Health Services provide services and support for foster care, residential and ACS involved youth..

Key Responsibilities

Provides general medical care and treatment to individual supported per program, on site.

Performs physical examinations and preventative health measures within prescribed guidelines and instructions.

Provide health evaluations, assessments, and screenings with follow-ups.

Review all Pediatric guidelines for care, including immunizations and testing

Records physical findings based on the patient’s condition.

Participate in clinical team and other health professionals to prepare comprehensive patient care plan utilizing historical and current information

Conducts and assists in medical examinations of youth after incidents.

Provides health education to youth supported, including reproductive health education

Administer Medication

Train staff

Assist with community provider visits as needed

Other related duties as assigned by supervisor.

Required Qualifications

Current Licensed Practical Nurse (LPN) license in New York State, in good standing.

Minimum of 2 years of nursing experience in a clinical, community-based, or related healthcare setting.

Current Basic Life Support (BLS) certification required.

Demonstrated ability to perform comprehensive nursing assessments and exercise sound clinical judgment in an autonomous work environment.

Strong critical thinking and problem-solving skills, with the ability to evaluate and interpret medical records, laboratory reports, and other clinical documentation.

Experience triaging patient calls, assessing urgency, and coordinating appropriate follow-up care.

Ability to provide guidance and support staff as needed while maintaining high standards of patient care.

Full-time position-hours are 7am – 3pm. Work is conducted in clinical settings within program facilities and residential programs. This is an in-person position only
