The Care Navigator assists unit leadership in assessing patient functioning and implementing comprehensive treatment plans. They provide direct care services, document patient responses, and collaborate with the multidisciplinary team to ensure access to required services.
Minimum of 2 years of experience in Human Services related field, medical related field, customer service, client/patient interfacing experience required OR Associate’s degree required, bachelor’s degree preferred.
Provides direct care services to patient on an individual and group basis, to include but not limited to activities of daily living, social, and psychiatric rehabilitation per treatment plan, as directed by Unit Leadership. Participates in the development of a comprehensive treatment plan sharing patient’s needs and wants as communicated by patient directly through patient surveys, collaterals, and treatment team. Documents and communicates patient response to treatment effectively and accurately and in a timely manner according to Program Policies and Procedures. Provides comprehensive care to patients in a professional and caring manner and is able to take appropriate actions and communicate findings to the multidisciplinary team. Participates in the peer review process and utilizes quality improvement techniques to improve quality and reduce costs. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops. Understands and practices established RGHS policies, procedures, goals, standards of care, quality improvement activities, safety, and environmental and infection control. PAY RANGE: $20.00 - $26.00 CITY: Clifton Springs POSTAL CODE: 14432 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
