The Care Navigator assists unit leadership in assessing patient functioning and implementing comprehensive treatment plans. They provide direct care services, document patient responses, and collaborate with the multidisciplinary team to ensure access to required services.

  • SUMMARY: Assist Unit Leadership in assessing patient’s functioning as well as developing and implementing treatment plans and services as patient’s needs warrant.

Minimum Qualifications

Minimum of 2 years of experience in Human Services related field, medical related field, customer service, client/patient interfacing experience required OR Associate’s degree required, bachelor’s degree preferred.

Responsibilities

Provides direct care services to patient on an individual and group basis, to include but not limited to activities of daily living, social, and psychiatric rehabilitation per treatment plan, as directed by Unit Leadership. Participates in the development of a comprehensive treatment plan sharing patient’s needs and wants as communicated by patient directly through patient surveys, collaterals, and treatment team. Documents and communicates patient response to treatment effectively and accurately and in a timely manner according to Program Policies and Procedures. Provides comprehensive care to patients in a professional and caring manner and is able to take appropriate actions and communicate findings to the multidisciplinary team. Intercedes on behalf of customer to ensure they have access to required services. Participates in the peer review process and utilizes quality improvement techniques to improve quality and reduce costs. Participates as a member of multidisciplinary team by providing feedback, maintain professional relationships, providing program and patient specific feedback and recommendations. Enhances professional growth and development through participation in educational programs, current literature, in-service meetings and workshops. Understands and practices established RGHS policies, procedures, goals, standards of care, quality improvement activities, safety, and environmental and infection control. EDUCATION: AS (Required) LICENSES / CERTIFICATIONS: PHYSICAL REQUIREMENTS: S - Sedentary Work - Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. PAY RANGE: $20.00 - $26.00 CITY: Clifton Springs POSTAL CODE: 14432 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components. Requirements
