About the role

The Nurse Practitioner will perform critical clinical assessments for patients with peripheral artery disease and collaborate with interventionalists to develop individualized vascular care plans. They are also responsible for maintaining accurate patient records and educating patients on disease prevention and treatment pathways.

We achieve this by empowering our team to raise awareness, eliminate pain, avoid unnecessary hospitalizations, and give patients their lives back. If you are a motivated, compassionate professional dedicated to treating some of the country’s sickest patients while lowering overall healthcare costs, join us. We are seeking a Full-Time Nurse Practitioner to join our growing outpatient vascular team. In this role, you will perform critical clinical assessments for patients with peripheral artery disease and other complex vascular conditions. You will work side-by-side with our expert interventionalists to build and execute clinical plans that restore blood flow to patients' limbs and prevent amputations. Key Responsibilities

Clinical Assessments: Conduct detailed physical exams and clinical assessments, focusing primarily on geriatric patient populations.

Care Planning: Partner with interventionalists to develop, implement, and manage individualized vascular care and pain management plans.

Documentation: Generate and finalize your own comprehensive visit notes using voice recognition software within our Electronic Medical Records (EMR) system.

Collaboration: Work within a multidisciplinary team to coordinate care and ensure strict adherence to patient safety protocols.

Patient Education: Educate patients and families on disease prevention, health promotion, and treatment pathways to support informed decision-making.

Quality Improvement: Maintain precise patient records and participate in clinical initiatives to enhance patient outcomes and safety.

Minimum Qualifications

  • Licensure: Licensed as a Nurse Practitioner in the state of New York (Required).
  • Education: Master’s or Doctoral degree in Nursing from an accredited program.
  • Certification: Board certification as a Family Nurse Practitioner (FNP) or equivalent specialty.
  • Experience: Minimum of 3 years of clinical experience in family medicine, vascular care, or a related healthcare setting.
  • Life Support: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.

Preferred Qualifications

  • Specialty Experience: Prior experience in wound care or pain management.
  • Lifespan Care: Additional certification or strong background in geriatrics.
  • Tech Savvy: Familiarity with EMR voice-recognition documentation and telehealth platforms.
  • Environment: Experience working in fast-paced, multidisciplinary surgical or outpatient settings.

Requirements

Candidates must hold a Master’s or Doctoral degree in Nursing and be a licensed Nurse Practitioner in New York with board certification. A minimum of 3 years of clinical experience in family medicine or vascular care is required, along with current BLS and ACLS certifications.
