Perform Physical Therapy evaluation and development of the plan of care appropriate for each patient based on the patient’s physical, mental, emotional, and developmental impairments and functional limitations. A variety of diagnoses and settings served include but are not limited to the 3 outpatient locations. Provide Physical Therapy interventions in accordance with established standards of care, policies, procedures, and state/federal law. Supervise Physical Therapist Assistant and Rehab Aides. Supervise and complete appropriate documentation for Physical Therapy and Physical Therapy Assistant students as assigned by CCCE. Complete documentation in electronic medical record in a timely manner in accordance within department and professional expectations demonstrating the need for skilled interventions. Participate in patient care conferences/ staffing as appropriate for multidisciplinary patients.
Requires a degree from an accredited physical therapy program ranging from a bachelor's to a clinical doctorate depending on graduation year. Must hold current IA and IL physical therapy licensure and CPR certification.
Education: Bachelor’s degree from an accredited Physical Therapy program prior to 2005 Master’s degree from an accredited Physical Therapy program between 2005-2019 Clinical Doctorate from an accredited Physical Therapy program 2020-Present License(s)/Certification(s): Current IA and IL Physical Therapy licensure (or eligible) upon hire Current CPR certification. License(s)/Certification(s): Current IA and IL Physical Therapy licensure (or eligible). Current CPR certification. *Specific offers are determined by various factors, such as experience, skills, internal equity, and other business needs.
