Responsibilities

Reports To: Office Manager Position Objective: The Licensed Practical Nurse (LPN/LVN) is responsible for providing direct patient care under the supervision of a Physician, Physician Assistant, APRN, or other licensed healthcare provider, in accordance with applicable state regulations. This safety-sensitive position plays a vital role in delivering high-quality, patient-centered care by supporting clinical operations, performing nursing procedures, assisting with diagnostic testing, administering medications, and collaborating with providers and care teams to promote positive patient outcomes within a Medicare Advantage, value-based care model focused on quality outcomes, and proactive patient engagement. Essential Responsibilities: Patient Care: Provide direct and indirect patient care under the direction of a VIPcare provider. Welcome patients and prepare them for examination by obtaining and documenting medical histories, chief complaints, current medications, allergies, surgical history, family history, social history, preventive care information, and other pertinent clinical data in the electronic medical record (EMR). Measure, document, and report vital signs and other clinical observations. Explain treatment plans and procedures to patients and prepare examination rooms prior to provider visits. Assist providers during examinations and procedures by preparing equipment and handing instruments as directed. Administer medications, injections, immunizations, and treatments within the scope of licensure and provider orders. Perform wound care, dressing changes, irrigation procedures, and other nursing interventions as appropriate. Initiate and monitor IV therapy, when certified and permitted by state regulations. Draw blood, collect specimens, and prepare laboratory samples for processing. Provide patient education and reinforce provider instructions regarding medications, treatment plans, and preventive care. Support Annual Wellness Visits (AWVs) by facilitating Health Risk Assessments (HRAs), reviewing and updating preventive care records, and ensuring all required documentation is complete prior to the provider encounter. Clinical Procedures & Diagnostic Testing: Perform CLIA-waived laboratory testing according to established protocols. Conduct in-office diagnostic testing, including but not limited to: Electrocardiograms (EKGs) Vision screenings Pulse oximetry Peak flow testing Oxygen administration as ordered Inhalation treatments Spirometry Perform PT/INR testing using approved equipment (CoaguChek/Coag-Sense) and glucose testing utilizing a glucometer. Complete required laboratory quality controls, maintain testing logs, and ensure compliance with applicable regulatory standards. Clinical Operations: Accurately document all patient care activities, treatments, test results, and supplies utilized within the EMR. Clean, disinfect, and sterilize reusable medical instruments and equipment according to infection prevention standards. Monitor inventory levels of pharmaceuticals, medical supplies, and clinical equipment, and assist with ordering as needed. Screen incoming patient calls and appropriately triage or route clinical concerns. Complete outbound patient communications, including Transitional Care Management (TCM) — Medicare-supported outreach within 2 days of hospital or facility discharge to reduce readmissions — preventive care outreach, medication adherence follow-up, and care coordination activities. Support front office operations as needed, including: Patient registration and check-in/check-out Prescription refill coordination Referral processing Telephone communication Other administrative support functions Team Collaboration: Collaborate effectively with providers, clinical staff, front office personnel, and interdisciplinary teams to deliver exceptional patient care. Maintain patient confidentiality and comply with all HIPAA and organizational privacy requirements. Participate in value-based care quality initiatives, including HEDIS measure tracking, Hierarchical Condition Category (HCC) coding support, and Medicare Advantage Star Ratings improvement activities. Perform other duties as assigned. Position Requirements/ Skills: High school diploma or equivalent required. Graduate of an accredited Licensed Practical Nurse (LPN/LVN) program. Current, unrestricted LPN/LVN license in the state of practice. 1+ year(s) of clinical nursing or ambulatory care experience preferred. Current CPR/BLS certification required or obtained within organizational guidelines. IV Therapy certification preferred, where permitted by state regulations. Phlebotomy experience preferred, including specimen collection and processing. Knowledge of nursing principles, clinical procedures, and patient care practices. Working knowledge of medical terminology, pharmacology, and common disease processes affecting the organization's patient population. Understanding of infection prevention, OSHA, CLIA, and HIPAA requirements. Familiarity with electronic medical records (EMRs) and clinical documentation standards. Knowledge of medical coding principles preferred. Proficiency with Google Workspace (Drive, Docs, Sheets, Slides). Excellent organizational, prioritization, and time-management skills. Strong critical thinking and clinical decision-making abilities. Exceptional verbal and written communication skills. Demonstrated ability to establish positive relationships with patients, providers, health plans, and internal and external partners. Ability to work independently while effectively collaborating within a multidisciplinary healthcare team. Strong attention to detail with the ability to maintain confidentiality of sensitive information. Demonstrated commitment to delivering high-quality, compassionate, patient-centered care. Appreciation for cultural diversity and sensitivity to the needs of Medicare and other patient populations served by VIPcare. Familiarity with Medicare Advantage programs, HEDIS quality measures, value-based care principles, Transitional Care Management, and Annual Wellness Visit workflows preferred. Physical Requirements: Ability to perform a full range of physical activities required in a clinical environment, including assisting with patient positioning and transfers. Frequent standing, walking, bending, kneeling, reaching, pushing, pulling, and repetitive hand movements throughout the workday. Manual dexterity and fine motor skills sufficient to administer medications, perform clinical procedures, and operate medical equipment. Ability to lift, carry, push, or move medical equipment and supplies weighing up to 15 pounds and assist with heavier patient transfers when appropriate. Ability to sit or stand for extended periods based on operational needs. Visual acuity and hearing (with or without reasonable accommodation) sufficient to safely perform essential job functions. Ability to operate standard office equipment, computers, telephones, and clinical devices. Reliable transportation to consistently report to scheduled work locations and shifts. Requirements
