Deliver hands-on physical therapy treatments to residents in assisted and independent living communities under the direction of a Physical Therapist. Collaborate with care teams to implement care plans focused on improving mobility, safety, and functional independence.

Responsibilities

Deliver hands-on treatments to residents in Assisted Living & Independent Living Communities under the direction of a Physical Therapist Carry out established care plans focused on mobility, safety, and functional independence Lead engaging therapy sessions using therapeutic exercise, functional mobility, balance activities, and manual techniques Help patients improve strength, balance, and overall quality of life Collaborate with care teams, staff, and caregivers to achieve strong patient outcomes Document treatments and patient progress in accordance with established care plans

Qualifications
