  • Description: Certified Nursing Assistant (CNA) – PACE About Trinity Health PACE Alexandria Trinity Health PACE Alexandria is a Program of All-Inclusive Care for the Elderly (PACE) in Central Louisiana that provides comprehensive medical, social, and supportive services that enable older adults to remain safely in their homes and communities. Through an interdisciplinary team approach, PACE coordinates medical care, rehabilitation, transportation, home care, social services, and caregiver support to promote independence, dignity, and quality of life for older adults. Trinity Health PACE Alexandria is seeking compassionate, dependable, and mission-driven professionals who are passionate about serving seniors and helping them thrive in the community.

Position Summary

The Certified Nursing Assistant (CNA) provides direct participant care and supportive services as part of the interdisciplinary team (IDT). The CNA assists participants with activities of daily living, promotes participant comfort and safety, observes and reports changes in condition, and supports participants receiving services in the PACE center, home, and community settings. Essential

Duties and Responsibilities

Participant Care Assist participants with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility. Assist with transfers, ambulation, and positioning using appropriate safety techniques and equipment. Provide restorative and supportive care that promotes independence and dignity. Assist with participant meals, hydration, and nutritional support as directed. Maintain participant comfort, privacy, and confidentiality. Monitoring and Reporting Observe participants for changes in physical, cognitive, emotional, or behavioral status. Promptly reports concerns, incidents, and changes in condition to the supervising nurse. Document services provided in accordance with program standards and regulatory requirements. Day Center Support Assist with participant arrival, departure, and transportation coordination. Support participant engagement in recreational, social, and therapeutic activities. Maintain a clean, safe, and organized participant care environment. Assist with set-up and clean-up of participant activities and meal services. Home and Community Support Provide participant assistance during home visits and community outings as assigned. Observe and report environmental or safety concerns identified in participant homes. Support participants during appointments and community-based activities as directed by the care team. Team Collaboration Participate in interdisciplinary team discussions as requested. Communicate effectively with nurses, therapists, social workers, drivers, and other team members. Support the implementation of participant-centered care plans. Demonstrate a commitment to the Trinity Health Mission, Core Values, and PACE philosophy of care.

Qualifications

Required Current Louisiana Certified Nursing Assistant (CNA) certification in good standing. High school diploma or equivalent. Current CPR/BLS certification or ability to obtain upon hire. Valid driver's license and reliable transportation. Ability to lift, transfer, and assist participants safely. Preferred Experience working with older adults in long-term care, home health, hospice, assisted living, adult day services, or PACE. Experience caring for individuals with dementia and chronic medical conditions. Knowledge of person-centered care principles. Knowledge, Skills, and Abilities Compassion for and commitment to serving older adults. Strong interpersonal and communication skills. Ability to work effectively within an interdisciplinary team. Ability to recognize and report changes in participant condition. Commitment to maintaining participant dignity, independence, and safety. Flexibility and adaptability in a dynamic care environment. Requirements
