Participant Care Assist participants with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, eating, and mobility. Assist with transfers, ambulation, and positioning using appropriate safety techniques and equipment. Provide restorative and supportive care that promotes independence and dignity. Assist with participant meals, hydration, and nutritional support as directed. Maintain participant comfort, privacy, and confidentiality. Monitoring and Reporting Observe participants for changes in physical, cognitive, emotional, or behavioral status. Promptly reports concerns, incidents, and changes in condition to the supervising nurse. Document services provided in accordance with program standards and regulatory requirements. Day Center Support Assist with participant arrival, departure, and transportation coordination. Support participant engagement in recreational, social, and therapeutic activities. Maintain a clean, safe, and organized participant care environment. Assist with set-up and clean-up of participant activities and meal services. Home and Community Support Provide participant assistance during home visits and community outings as assigned. Observe and report environmental or safety concerns identified in participant homes. Support participants during appointments and community-based activities as directed by the care team. Team Collaboration Participate in interdisciplinary team discussions as requested. Communicate effectively with nurses, therapists, social workers, drivers, and other team members. Support the implementation of participant-centered care plans. Required Current Louisiana Certified Nursing Assistant (CNA) certification in good standing. High school diploma or equivalent. Current CPR/BLS certification or ability to obtain upon hire. Valid driver's license and reliable transportation. Ability to lift, transfer, and assist participants safely. Preferred Experience working with older adults in long-term care, home health, hospice, assisted living, adult day services, or PACE. Experience caring for individuals with dementia and chronic medical conditions. Knowledge of person-centered care principles. Knowledge, Skills, and Abilities Compassion for and commitment to serving older adults. Strong interpersonal and communication skills. Ability to work effectively within an interdisciplinary team. Ability to recognize and report changes in participant condition. Commitment to maintaining participant dignity, independence, and safety. Flexibility and adaptability in a dynamic care environment.
