The Coordinator will advocate for and educate residents in long-term care facilities while mentoring volunteers. Responsibilities include visiting facilities, resolving complaints, and ensuring compliance with state regulatory requirements.

Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.

Schedule

21 hours per week- 7 hours a day x three days a week.

SUMMARY

Family Service League is seeking a part-time Long Term Care Coordinator for our Ombudsman program in Bay Shore, NY. The Coordinator will empower, educate, and advocate for residents in licensed long term care facilities in Suffolk County. Responsibilities include mentoring volunteers, visiting uncovered nursing homes, assisted living , adult homes and family type homes.

Responsibilities

  • The Coordinator will visit all long term care facilities that do not have an Ombudsman assigned to it to ensure residents have access to an Ombudsman.
  • Travel to various long-term care facility locations throughout Suffolk County to provide coverage and mentor Ombudsman volunteers.
  • Escalate to Program Coordinator of critical issues and lack of resolution to complaints.
  • Assist residents and family members to develop councils.
  • Accurately complete all reports and documentation required by the program and submitting them by the required deadline.
  • The Coordinator will attend staff meetings, in-service trainings, and program trainings.
  • Participates with the certification training of new Ombudsman volunteers.
  • Assure compliance with Family Service League policies, standards and regulatory requirements including the Office of the State Long Term Care Ombudsman and New York State Department of Health.
  • The Coordinator will interact effectively with service provider agencies, advocacy organizations and regulatory agencies.
  • Provides resources and referral information to residents and families and assists them with navigating the long-term care system.
  • Demonstrates cultural competency with communications and complaint resolution.
  • Obtain NYS Ombudsman certification.
  • All other duties as assigned.

Qualifications

Bachelor’s Degree in human services, social work or related field of study required.

NYS Certified Ombudsman certification required - training provided and must be completed within 6 months.

Requirements

A Bachelor's degree in human services, social work, or a related field is required, along with a valid New York State driver's license. Candidates must obtain NYS Ombudsman certification within six months and possess at least three years of experience working with older adults or disabled individuals.
