Position Summary

(Overall purpose of this position) Under the direction of a provider or registered nurse: Support providers in the examination and treatment of patients, including the preparation of exam rooms and instrumentation, operation of medical equipment, administer medications and injections, etc. related to patient care requirements. Essential duties and responsibilities which must be performed in order to carry out the position purpose summarized above: (The following description is a general representation of the key duties and responsibilities of this position. Other duties may be assigned, as required.) Establish reason for visit and document patient description of chief complaint. Document all EPIC visit encounter fields and smartphrases. Obtain baseline data on all patients including histories, vital signs, health education and lab tests as indicated by the patient presentation. Assure all required blood work and back office diagnostic labs are completed and documented in the EMR before provider exam room visit. Assure provision of all instruments, order labs and vaccines and document for physicals and well child visits. Evaluate patient problem and implement patient care plan as established. Follow-up on provider post exam requests; dress wounds, perform ear irrigations, recheck blood pressure etc. Administer medications/immunizations, evaluate effectiveness. Assure patient is informed about possible effects. Assure the timely and accurate documentation of patient care observations and activities. Provide ongoing support to patients regarding specific health or psychosocial problems. Utilize established guidelines and educational materials. Evaluate patient knowledge regarding health, illness and self-care practices; promote positive lifestyle and health habits. Develop and teach classes on health topics to patients and community residents as requested by clinic. Maintain patient flow for providers, filling rooms per designated quota. Clean and stock rooms per established requirements. Maintain quality controls daily. Order supplies, immunizations and provide assistance as directed or needed. Demonstrated proficiency in utilizing the EMR system for documenting patient care, reviewing the record for health maintenance needs, entering orders, medication refill requirements, scheduling appointments, and processing the in-basket folders and messages. Collaborating with public health and social service agencies regarding patient health issues. Serve as a member of a Patient Center Medical Home team (PCMH); confer with team members and appropriate community resources. Provide telephone support: evaluate patient problem and assure optimal outcomes. Maintain updated knowledge of telephone care practices and clinical skills in disease management and health promotion. QualificationsQualifications: Specific knowledge, training or skills required to perform the duties of this position. Specific concepts, courses, training programs or required certifications: (To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Knowledge of injection levels and procedures (children and adults). Knowledge of universal precautions, bloodborne pathogens and OSHA guidelines Demonstrated understanding of scope of practice and clinical nursing skills. Demonstrated understanding of community health and interest in teaching patients, families and groups. Demonstrated critical thinking skills and ability to work independently. Demonstrated commitment to continuing professional development through participation in continuing education programs. Demonstrated computer proficiency, EMR. Demonstrated interpersonal skills, cultural competency and understanding of underserved populations. Demonstrated team membership skills. Demonstrated ability to walk, sit or stand for long periods of time. Requires some lifting up to 50 lbs. Work environment is stressful at times. Previous experience that is necessary background to qualify for this position: Moderate level of broad, general nursing experience. REPORTS TO AND EVALUATED BY: Manager of Nursing Department

Requirements

Candidates must hold a valid Licensed Practical Nurse license in the State of Ohio and possess knowledge of injection procedures and OSHA guidelines. Strong interpersonal skills, proficiency with EMR systems, and the ability to work independently in a clinical setting are required.
