Mobile Crisis Coordinator

At Easterseals PORT Health (ESPH), our mission is rooted in empowering individuals and strengthening communities. We’re seeking a compassionate and dedicated full-time Mobile Crisis Coordinator to join our Community Regional Crisis Response (CR2) team in Virginia. You must live in Lynchburg, Bedford, or Appomattox areas, and be licenses eligible.

Schedule

Full-Time: (3) 12 hour shifts 9P-9A and (1) 4 hour shift, Sun- Wed

Management reserves the right to assign or reassign duties and responsibilities to this job at any time.

Position Summary

The Reach Mobile Crisis Coordinator functions primarily to provide clinical and coordination of services for REACH Mobile Crisis Team. Primary duties include on-site and telephonic/telehealth crisis intervention to include a shared rotation of a 24/7/365 crisis line. Primary duties also include crisis stabilization for individuals who are temporarily accessing crisis services within the individual’s home or other community setting.

Essential job functions

· Provides rapid response to individuals experiencing a behavioral health crisis.

· Meets the individual in an environment where they are comfortable to facilitate service engagement, stabilization, and resolution of the crisis when possible

· Provides appropriate care/support/supervision in order to maintain safety for the individual and others, including but not limited to crisis intervention, community stabilization, care coordination, and treatment planning

· Refers and links to all medically necessary behavioral health services and supports

· Coordinates with behavioral health providers providing services to the individual throughout the delivery of the service

· Maintains linkages and relationships with community partners.

· Ensures the completion of all required documentation for Mobile Crisis services, including but not limited to assessment, treatment planning, and Crisis Education and Prevention Plans (CEPP).

· Participates in required meetings with REACH Team Members and other applicable parties.

· Coordinates admissions/discharges and requests for appropriate individuals to/from REACH Crisis Therapeutic Home in collaboration with the REACH CTH Supervisor

Requirements

Candidates must hold at least a bachelor's degree in a human services field and have one year of relevant experience, with a preference for a master's degree and professional licensure. Applicants must be eligible for or hold a QMHP-T registration and possess a valid driver's license.

What we are looking for

· Qualified Mental Health Professional (QMHP) and/or Qualified Developmental Disabilities Professional (QDDP)– Minimum of a bachelor’s degree in human services or related field from a USDOE or CHEA accredited institution and no less than one year of fulltime relevant experience.

· Graduation from an accredited college/university with a Master’s Degree in Counseling, Social Work, Psychology, or closely related field preferred.

· Current, unrestricted Virginia license as an LPC, LCSW, or LMFT or closely related preferred.

· Must be registered (or eligible to register) as a QMHP-T with the Virginia Board of Counseling or hold a valid Licensed Mental Health Professional-Resident or Supervisee.

· Those not currently registered must be eligible to apply and must submit their QMHP-T application upon hire, with registration obtained within 30 days, unless they are a Licensed Mental Health Professional.

· Experience in intellectual/developmental disabilities, including programming for individuals with significant behavioral needs preferred.

· Knowledge of the MH and I/DD systems of care.

· Knowledge and ability to utilize computer word processing and data programs including but not limited to Microsoft Office Suite, Web-based data entry, etc.

· Must demonstrate capacity to effectively communicate verbally and in written form.

· Must have a valid, unrestricted, state appropriate driver’s license and vehicle insurance.
