What You Will Do: Conduct daily Start of Care (SOC) visits and comprehensive patient assessments in the home Develop, implement, and evaluate individualized plans of care in collaboration with physicians and the interdisciplinary care team Complete the admission process, including obtaining informed consent, reviewing patient rights, advance directives, emergency preparedness, and agency policies. Accurately complete OASIS documentation and ensure compliance with Medicare, Medicaid, state, and agency regulations Communicate assessment findings, admission orders, care coordination needs, visit frequencies, and identified risks to the Clinical Management team and physicians via Admission report. Provide skilled nursing care, including medication administration, wound care, treatments, and patient education Educate patients and caregivers on disease management, medications, home safety, and the home health plan of care Respond to urgent patient needs and clinical situations promptly and professionally
Current, active Registered Nurse (RN) license Valid driver's license and reliable transportation At least one year of nursing experience; home health and OASIS experience preferred Knowledge of OASIS, Medicare Conditions of Participation, and home health regulatory requirements.
