Choice Health at Home is seeking a compassionate and dedicated Physical Therapist to assess, plan, organize, and provide rehabilitative care in patients’ homes. In this role, you will help patients improve mobility, relieve pain, increase strength, restore physical function, and manage disabling conditions resulting from illness or injury.
Key responsibilities
Perform and document comprehensive initial evaluations, analyze clinical findings, identify functional limitations, and establish a diagnosis and plan of care before beginning treatment.
Develop and implement individualized treatment programs designed to maintain, improve, or restore physical function, alleviate pain, and prevent further dysfunction.
Evaluate patients’ responses to treatment at various stages and modify interventions as needed to achieve the greatest possible benefit.
Apply infection-control practices and universal precautions while providing patient care.
Ensure continuity and quality of care through accurate, timely, and complete documentation.
Communicate effectively with patients, families, caregivers, members of the interdisciplinary team, and other healthcare professionals.
Educate patients and caregivers on treatment plans, therapeutic exercises, safety practices, and strategies for achieving functional goals.
Uphold the agency’s philosophy, clinical standards, and administrative policies.
Complete all required documentation within established company guidelines.

Candidates must have successfully completed an accredited physical therapy program and hold a current state license. A valid driver's license, automobile insurance, and CPR certification are required, with home health experience preferred.
