As an RN Case Manager, you will support long-term clients age 60 and older enrolled in the MyCare Ohio CareSource waiver programs through clinical assessment, care coordination, advocacy, and ongoing case management. This opportunity combines healthcare leadership, community impact, and hybrid field-based flexibility while improving quality outcomes for older adults and individuals with disabilities.
