  • SUMMARY: Under the supervision of the Director of Nursing, the Registered Nurse/ Licensed Practical Nurse/ Charge Nurse provides quality nursing care to meet residents needs, coordinates and conducts activities of daily living, addresses family/social concerns, documents and contributes to health status assessments and care plans, administers medications, and performs skilled treatments and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES This summarizes the major essential duties and responsibilities regularly performed and normally assigned to this position.

Listed below are the duties in descending order of importance and are as clear and specific as possible.

Other duties may be assigned.

May be necessary to assist in the evacuation of residents during emergency situations.

Provides daily supervision to assigned staff and directly provides nursing care to residents in Skilled Nursing Unit, Assisted Living Unit, Dementia Unit, as assigned.

Participates in the admission of new residents as needed.

Assists residents with Activities of Daily Living (ADL), family/social activities and actively promotes independent life-style of residents.

Responds to residents call system promptly and courteously.

Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.

Provides daily report to the Supervisors concerning environmental conditions, personnel performance, resident conditions and needs for nursing supplies.

Consistently demonstrates hospitality behavior and the core values of quality, respect, service and integrity.

Provides nursing care according to physician’s orders and in conformance with recognized nursing practice, established standards, and administrative policies.

Ensures preventative, therapeutic and rehabilitative services to residents and assists in the coordination of nursing care with care rendered by other disciplines.

Makes rounds with physicians if needed, assists in examinations and/or treatment of residents and carries out orders.

Has the skill to assess resident and notify attending physician and responsible party in the event of a change in the resident’s condition.

Accurately administers prescribed medications, supervises the continuity of nursing care, responds to changes in medical conditions, medications or other circumstances requiring special care needs and performs first aid as required.

Provides daily supervision and leadership to assigned staff and other designated personnel in meeting the resident’s needs.

Demonstrates good decision making skills.

Provides written evaluation and oral counseling of assigned staff if needed.

Assesses and monitors complex resident health status and determines the appropriate nursing intervention.

Orients residents and staff to emergency procedures and responds to all emergency situations.

Completes and signs incident reports and provides follow-up as required.

Assures that all resident records are complete and accurate and that all mandatory forms are on file.

Through familiarity and application of facility policies and procedures, assures that nursing care is practiced in full accordance with regulatory requirements, the Meadows quality standard and resident rights.

Monitors resident environment to ensure all safety and sanitary regulations and quality standards are met or exceeded.

May assist with development and presentation of in-service programs.

May serve on committees, as requested.

Attend unit meetings and all mandatory and optional in-services.

Qualifications

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. EDUCATION and/or EXPERIENCE

Associate's degree (A. A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.

Language skills

Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

Mathematical skills

Ability to calculate figures and amounts such as discounts, interest, commissions, proportions, percentages, area, circumference, and volume. Ability to apply concepts of basic algebra and geometry.

Reasoning ability

Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in, written, oral, diagram, or schedule form.

*computer skills

Proficiency with Microsoft software (Word, Excel, etc.). Familiar with telephone equipment and various other office equipment.

State of Pennsylvania license as a Registered Nurse or Licensed Practical Nurse.

Other skills and abilities

Ability to effectively manage multiple priorities. Ability to work in a fast paced and quick changing environment. Ability to adapt to software designed for electronic medical record.

Physical demands

While performing the duties of this job, the employee is required to talk, hear, stand, walk, sit, use hands to finger, handle or feel objects, and reach with hands and arms. The employee must be able to lift and/or moves up to 25 pounds. The employee must also be able to push a minimum of 200 pounds and be able to push such weight a minimum distance of 200 feet. Specific vision abilities required by this job include close vision, distance vision and the ability to adjust focus.

The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee will work in a clean, well-lighted, well-ventilated building. The noise level in this environment ranges from quiet (private office) to moderate (business office with computers and printers, light traffic).
