- ROLE: The Case Manager provides comprehensive case management services to individuals and families at risk of homelessness. Responsibilities include conducting assessments, developing service plans, connecting participants to housing and community resources, assisting with benefits and financial assistance applications, and supporting clients in achieving long-term housing stability and self-sufficiency. ESSENTIAL DUTIES & RESPONSIBILITIES Provide case management services to individuals and families at risk of homelessness. Conduct assessments and develop individualized service plans to address housing barriers. Connect participants to housing resources, benefits, and supportive services. Assist clients with financial assistance applications and benefit enrollment or reinstatement. Advocate for clients and coordinate services with community providers and landlords. Monitor participant progress and maintain accurate case records and documentation. Support clients in achieving housing stability, self-sufficiency, and long-term success. Collaborate with internal and external partners to ensure participants receive comprehensive services. Other duties as assigned.

Qualifications
Requirements
HS Diploma, GED, or AA degree in related field and two years qualifying experience PREFERRED QUALIFICATIONS & SKILLS MSW or bachelor’s degree preferred in related field. Knowledge of the Department of Homeless Services, the Human Resources Administration, and entitlements. Knowledge of the complex interaction of multiple factors of homelessness, incarceration, and substance abuse, mental health and domestic violence is preferred with sensitivity to cultural and gender specific issues with both individuals and families.
