Assist individuals with daily living activities, including personal care, meal preparation, housekeeping, and hygiene support.
Promote independence and help individuals achieve their personal goals and highest quality of life.
Provide compassionate, person-centered support in residential settings.
Support community participation, relationship-building, and meaningful social engagement.
Transport individuals to medical appointments, community activities, day programs, religious services, and family visits.
Administer and document medications in accordance with organizational policies and procedures.
Complete required documentation including daily notes, medication logs, incident reports, behavior tracking, and goal progress reports.
Assist with Individual Service Plan (ISP) implementation and outcome tracking.
Monitor for health and safety concerns and respond appropriately in emergency or crisis situations.
Implement behavior support plans and trauma-informed care practices.
Maintain client confidentiality and adhere to HIPAA requirements.
Participate in trainings, staff meetings, and agency initiatives.
Ensure the safety, well-being, dignity, and rights of all individuals served.
Collaborate with the Residential Manager, Lead DSP, families, and interdisciplinary team members to support individual success.
Perform other duties as assigned to meet the needs of the program and individuals served.

Physical requirements
Ability to lift, transfer, and support individuals as necessary, including lifting up to 50 pounds.
Ability to stand, walk, bend, stoop, reach, and climb stairs throughout a shift.
Ability to assist with personal care and daily living activities.
Ability to operate agency vehicles and transport individuals safely.
Ability to perform essential job functions with or without reasonable accommodation.
