Wellness RN
The Wellness RN performs annual wellness visits, develops preventive care plans, and manages care for high-risk and ACO patients. They also provide direct nursing care including triage, treatments, and laboratory services while maintaining compliance with safety and quality standards.

What you’ll do
Performs Annual Wellness Visits, develops preventive care plans, and documents per CMS and provider guidelines.
Manages care for high-risk and ACO patients, coordinating services and participating in daily/multidisciplinary huddles.
Provides direct nursing care including triage, treatments, injections, labs, and patient admissions per protocol.
Maintains compliance with licensure, safety, infection control, and quality improvement standards.
Collaborates across departments, supports organizational mission, and upholds CommWell Health’s core values.

Candidates must hold a current North Carolina Registered Nurse license and a BLS certificate from the AHA or Red Cross. A minimum of one year of experience in nursing or a related medical field is required, along with a clean driving record.
