Social Worker (MSW)

Job Description

KEY ACCOUNTABILITIES: Identifies patients with complex care needs, conducts patient psychosocial status assessment, establishes a rapport with patient/family, and actively participates in creating and implementing a plan of care to achieve outcomes. Utilizes effective oral and written communication skills to collaborate with the health care team as needed to enhance quality and care. Develops psychosocial plan of intervention for the patient and family utilizing resources within the community. Incorporates social, emotional, financial, and spiritual aspects of care into the patient’s plan. Evaluates progress toward outcomes/goals for individual plan for patient and modifies plan as appropriate. Serves as a consultant, patient advocate, and liaison for patients and families in areas of social work needs and complex psychosocial issues that could include facilitation of specialty-specific psychoeducational support groups in some care settings. Arranges for resources/funds to obtain medications, DME, housing, transportation, and other needed services. Facilitates appropriate and prompt access to primary care providers, physician specialists, and similar services as necessary. Coordinates patient information and communication between and among the patient/family and the referring/accepting facilities, and communicates to caregivers as applicable to ensure smooth transition of care. Advocates for the patient and performs discharge planning/after care coordination. Supports efforts to decrease hospital readmissions when possible and returns patient to the community with appropriate resources. Facilitates advanced care planning and lifespan planning.

Qualifications

Master’s degree in social work Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW) American Heart Association Healthcare Provider Basic Life Support (BLS) PHYSICAL DEMANDS: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to a maximum of twenty-five (25) pounds without assistance Job classification is exposed to blood borne pathogens (blood or bodily fluids) while performing job duties WORK ENVIRONMENT: Frequent exposure to sharp objects and instruments Occasional exposure to moving mechanical parts, fumes or airborne particles, toxic or caustic chemicals, and risk of electrical shock Occasional high noise level in work environment Possible exposure to communicable diseases, hazardous materials, and pharmacological agents Occasional contact with aggressive and or combative patients .
