About the role

The Case Manager is responsible for maintaining high-quality clinical records and developing individualized service plans for clients. They provide community psychiatric supportive treatment, crisis intervention, and therapeutic behavioral services to support client functioning and recovery.

Harbor is seeking a CPST/TBS Provider to join the Bowling Green team. This position provides professional care to children and youth experiencing serious emotional disturbances (SED), adults experiencing a severe and persistent mental illness (SPMI), individuals with co-occurring mental health and substance use disorders (COD), and their families.

Position is 1st shift and full-time.

Requirements

  • Bachelor’s degree in Counseling, Social Work, Psychology, or a closely related behavioral/mental health field required, or, in lieu of a degree, a minimum of a high school diploma and 3 years of relevant experience.
  • Must be proficient and accurate in computer use, including Microsoft Word.
  • Must successfully complete required training, before services are rendered, in basic competencies including, at a minimum, an understanding of mental illness or substance use disorder treatment and recovery, psychiatric and substance use disorder symptoms and their impact on functioning and behavior; how to therapeutically engage a person with mental illness or a person in substance use disorder treatment and recovery;; crisis response procedures; an understanding of the community behavioral health system, social service systems, criminal justice system and other healthcare systems; de-escalation techniques and understanding how your own behavior can impact the behavior of others.
  • Must successfully demonstrate the basic competencies listed above, as determined by your supervisor’s review, within 90 days of hire.
  • Must have a valid driver’s license, acceptable driving record, and show proof of personal auto insurance policy to be deemed insurable with Harbor’s vehicle insurance carrier.
  • For specialized programs:
  • Developmental Pediatrics: Completion of a 40-hour Registered Behavior Technician course required or to be obtained within 120 day introductory period.
  • Substance Use Disorder Program: May require completion of a 40 hour State of Ohio Chemical Dependency Counselor Assistant certification.
  • When applicable, non-licensed staff will be required to complete 20 hours of CEUs every 24 months.
  • CPR/First Aid and NCI/CPI Certifications required within 90 days of employment.

Responsibilities

Documentation

1.

Responsible for the quality and completeness of the clinical record.

2.

Develops and updates Individualized Service Plan (ISP) for each client.

3.

Responsible for accurate and timely documentation which may include but is not limited to activity/progress notes, ISPs, incident report forms, and any other forms necessary to document services.

4.

Completes clinical documentation in the format appropriate and acceptable to Harbor 5.

In providing billable services, meets or exceeds unit expectations established by Harbor.

Provides Community Psychiatric Supportive Treatment service activities

1.

Evaluation 2.

Assesses client needs and formulates a strength-based ISP in conjunction with the client, family and/or significant others.

3.

Evaluates and revises the ISP according to the client’s responses and needs.

4.

Crisis prevention / intervention 5.

Follows crisis prevention/intervention protocols.

6.

Ongoing interventions 7.

Provides support, education and consultation to clients, families, and/or significant others.

8.

Provides therapeutic interventions as noted in the ISP to maintain the client in the least-restrictive environment and to enhance client and family functioning.

9.

Monitors clients to identify and minimize effects of psychiatric symptoms.

10.

Provides education on self-management of symptoms to clients, families and/or significant others.

Collaboration

1. Assists the client and family in increasing social support skills and networks. 2. Coordinates indicated evaluations and assessments by internal and/or external providers. 3. Coordinates and monitors all services identified in the ISP. 4. Provides assistance in gaining access to essential community resources.

Provides individual and group Therapeutic Behavioral services, including but not limited to:

1.

Treatment Planning.

2.

Identification of strategies or treatment options.

3.

Developing and providing solution focused interventions and emotional and behavioral management drawn from evidence-based psychotherapeutic treatments.

4.

Restoration of social skills.

5.

Restoration of daily functioning.

6.

Crisis prevention and amelioration.

7.

Group services.

Additional Mental Health Day Treatment expectations may include

1. Provides Therapeutic Behavioral Group services within a day treatment model that may include, but are not limited to, the following: Determination of needed mental health interventions; Skills development; Developing positive coping mechanisms; Managing mental health and behavioral symptoms to enhance vocational/school opportunities and/or independent living; Psycho-educational interventions including individualized instruction and training of persons served in order to increase their knowledge and understanding of their psychiatric diagnosis(es), prognosis(es), treatment, and rehabilitation.

Additional Developmental Pediatrics expectations may include

1. Collect data accurately and as prescribed by BCBA. 2. Organize and graph data as required by BCBA. 3. Carry out interventions, including ABA interventions, and teach caregivers to carry out intervention as prescribed in the behavior plan 4. Able to understand and carry out behavior plans as written with BCBA.

Additional Outreach Position expectations may include

1.

Provides CPST/TBS, day treatment, consultation, and other services as needed for individuals located at designated community location (e.g., Cherry Street Mission, schools).

Works on-site at designated community location.

Assists with program development and engagement in services at community location in coordination with supervisor.

Works with supervisor to arrange coverage as needed.

Special Work Conditions

1.

Keeps current with trends and developments related to essential job competencies and demonstrates continued growth in the essential job competencies.

2.

May float to other locations to accommodate client or agency need.

3.

Demonstrates fundamental alcohol and other drug addiction, abuse, and recovery competencies, at a minimum, an understanding of alcohol and other drug treatment and recovery, how to engage a person in treatment and recovery and an understanding of other healthcare systems, social service systems and the criminal justice system.

4.

Carries and responds to cell phone calls as part of general duties.
