Case Manager (Trans Services)
San Francisco Community Health Center is a Federally Qualified Health Center (FQHC) that provides culturally competent high quality medical care, mental health services, education, and outreach. Trans:Thrive is a safe space for community members to access programming to improve their quality of life. Offering case management, medical services (including Primary Care, HRT, referrals for gender affirming surgery), substance use services and treatment (including harm reduction supplies, counseling, treatment linkage), support groups, meals, and community events.
The Case Manager of Trans Services is primarily focused on providing case management services to eligible trans/GNC clients a part of
Trans:Thrive programs and the SF Community Health Center.
The fundamental tasks to perform are, but not limited to, the following: conducting client intakes and assessments, developing care plans, and providing care coordination with other internal and external providers including PCPs, RNs, CHWs and care coordinators. The Case Manager is also responsible for providing as needed health education, supportive counseling, crisis intervention, benefits advocacy, and coordination of services within the agency and between other agencies.
Candidates must possess a bachelor's degree in a health or social work-related field or equivalent experience, along with at least one year of experience in case management or outreach. Proficiency in harm reduction, trauma-informed care, and strong communication skills are required, as is the ability to work occasional weekends and evenings.
Bicultural, bilingual in Spanish (or other SF threshold language)
Experience working in a Federally Qualified Health Center (FQHC) or other type of community health centers.
Self-starter and self-motivated.
PC computer skills preferred, especially with MS Office products
Experience in providing social services or refugee assistance
Knowledge of EPIC, Excel, Zoom and being able to learn other computer programs related to administrative work.
