The Dementia Care Coordinator provides care coordination, respite, education, and resources to support individuals living with dementia and their families. This role involves managing documentation and utilizing care management systems to ensure high-quality support for patients.

Job DetailsJob Location

  • Newark, DE 19702Delaware Hospice is actively seeking a full-time Dementia Care Coordinator. Schedule will be Monday-Friday 8am-430pm. The Dementia Care Coordinator plays a key role in supporting the team in delivery of care coordination, respite, education, and resources to support people living with dementia, their families, and caregivers. This role would best be filled by someone that is compassionate, detail oriented, has strong communication and organizational skills, and can multi-task and prioritize independently. Qualifications Minimum of two (2) years of experience working in an environment utilizing basic medical practice knowledge Required Dementia Specific Education for Center for Medicare and Medicaid Services regulations (to be done at time of orientation and will be supplied by Delaware Hospice) Compassionate approach to supporting individuals, families, and caregivers navigating life-changing diagnoses Strong interpersonal, communication, documentation, and organizational skills. Comfort using care management systems, electronic health records, and other technology‑based tools.

Requirements

Candidates must have at least two years of experience in an environment utilizing basic medical practice knowledge. Strong interpersonal, communication, and organizational skills are required, along with comfort using electronic health records.
