The Case Manager meets with patients and families to explain the scope of hospice services, coordinates admission of the patient, and develops the ongoing plan of care.

Candidates must be a Registered Nurse in the state of Texas with at least one year of nursing experience. Hospice case management experience is required, with two or more years preferred, along with a valid driver's license.
