About the role

The Registered Nurse is responsible for planning and delivering skilled nursing care, including palliative and supportive services to patients and their families. They must perform comprehensive assessments, manage medications, and collaborate with the interdisciplinary team to update patient care plans.

Definition

The RN is responsible for the planning and delivery of patient care, providing skilled nursing care and oversight in accordance with policies and the Florida Nurse Practice Act, utilizing the nursing process of assessment, planning, intervention, implementation and evaluation. To provide a combination of skilled professional nursing tasks and provide palliative and supportive care to the patient and family unit.

Qualifications

 Current and active Florida RN license.

 Must be Certified Hospice and Palliative Registered Nurse (CHPN) within 12 months.

 Current and valid Florida driver’s license and automobile insurance.

 Current CPR certification required.

 Minimum of one year RN experience where physical assessment skills were utilized.

 Prior experience in oncology, home health, Hospice or palliative care setting is preferred.

 Must be able to handle sensitive issues appropriately using sound judgment, maintain professional boundaries, and maintain confidentiality.

 Must be able to effectively plan, organize and prioritize work.

 Must be self-directed and able to work independently in the field.

 Basic computer skills and EMR software experience preferred.

 Per Diem works at least one weekend per month.

 Attendance is a condition of employment.

Expectations

 Participate in departmental specific initiatives including but not limited to performance improvement projects, staff meeting attendance, employee training and development.

 Contribute to program development strategies for Treasure Coast Hospice, in an organized, routine capacity that will enable the cohesion of team developments.

 Work collaboratively with other departments, demonstrate and encourage leadership for a comprehensive approach to program development.

 Demonstrate effective communication with patients, families, and team members.

 Inspire staff to live the Treasure Coast Hospice’s Mission, to achieve its Vision, and demonstrate our Values and Commitment. Be current in legal and compliance hospice issues.

 To meet or exceed identifiable standards for the position.

Key Result Areas

 Responsible for upholding Treasure Coast Hospice/Model Standards for patient visits and compliant documentation.

Requirements

Candidates must hold an active Florida RN license, current CPR certification, and at least one year of experience utilizing physical assessment skills. A valid Florida driver's license and automobile insurance are required, and certification as a Hospice and Palliative Registered Nurse (CHPN) is expected within 12 months.

 Provide clinically professional direct patient care to include a variety of skilled nursing tasks to include, but not limited to, assessments, medication administration (PO/SL, Sub cue, IV, PR) wound care, ADLs.

 Administer medications according to policy and procedures, to include evaluating for response and effectiveness, and document accordingly.

 Administer medications which include controlled substances, in accordance to physician orders and policy and procedures.

 Destroy medications that are considered narcotic in nature and all other medication that is no longer being used in accordance with policy.

 Communicate to the IDT all pertinent information pertaining to the patient and family unit, actively to participate in review and revision of patients’ plan of care.

 Complete education requirements in a timely manner.

 Documentation should include but not limited to variance in patient condition, interventions and outcomes and address progress towards goals set by the patient’s plan of care.

 Documentation should include education provided to the family and the patient, including topic, which was taught and their understanding of the education.

 Participate as an active supporter of the Treasure Coast Hospice compliance program, including the emphasis to report, as well as complying with all Federal, State and Local laws as well as the organization’s compliance program.

 Supervise C.N.A.s and LPNs

 Fulfill all responsibilities related to the success of the strategic plan of the organization, as requested and assigned.
