Advanced Practice Provider
Provide primary care to patients under physician supervision and collaborate on medical protocols and operational policies. Assist in supervising clinic staff, managing patient registries, and participating in quality improvement activities.

Cahaba Medical Care Foundation is a community health center providing medical, pharmacy, dental, and behavioral health services to diverse underserved communities in Jefferson, Bibb, Perry, Chilton, Wilcox, Marengo, and Shelby counties. We are a Level 3 Patient-Centered Medical Home and Joint Commission accredited organization, committed to increasing integration and coordination of behavioral health and primary care. Requires certification as a Family Registered Nurse Practitioner or Physician Assistant with a valid state license. A minimum of two years of experience in a primary care setting is preferred.
