We're looking for a full-time Paneled Primary Care Nurse Practitioner who is passionate about caring for seniors, building long-term patient relationships, and managing a dedicated panel in a value-based primary care model.
This onsite position is located at our Homeland Park, South Carolina clinic and follows a Monday-Friday schedule.
You will report directly to the Associate Medical Director and work with an interdisciplinary care team to deliver high-quality, patient-centered care.

Manage primary care services to a designated panel of senior patients Improve panel performance through quality metrics (e.g., HEDIS, STARs, risk adjustment) and patient engagement plans Coordinate care across interdisciplinary teams, including specialists, behavioral health, and care management Report to the Associate Medical Director Work fully onsite in Homeland Park, SC Required Qualifications Graduate of an accredited Nurse Practitioner program (MSN or DNP) Current national certification (AANP or ANCC) Active, unrestricted NP license in South Carolina DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire) Experience We're Looking For 2+ years of Nurse Practitioner experience Experience managing a patient panel in a primary care Background in Medicare Advantage or geriatric-focused care Familiarity with HEDIS quality metrics, STAR ratings, and Medicare guidelines Work Environment Competitive compensation package and full benefits Outpatient clinical setting with a focus on continuity and preventive care Standard schedule: Monday–Friday, 8:00 AM – 5:00 PM #nursepractitionercareers #LI-JK1 Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
Requires a graduate degree from an accredited Nurse Practitioner program and an active, unrestricted NP license in South Carolina. Candidates must hold current national certification (AANP or ANCC) and possess at least 2 years of relevant experience.
