About the role

The Care Manager acts as a catalyst to promote care coordination, continuity of care, and efficient resource management for complex patients. They are responsible for managing patient care from pre-admission through immediate post-discharge to ensure consistent psychosocial outcomes.

Under supervision of Care Management Manager, works as a catalyst to promote the following: coordination of care, continuity of care, achievement of consistent and/or expected psychosocial outcomes, patient advocacy and efficient resource management. Coordinates the care for complex designated patients from pre-admission to immediate post-discharge.

Required Qualifications

  • Master's Degree in Social Work or related field
  • Upon hire: Washington Clinical Independent Social Worker Associate License
  • 3 years of Social services experience with a health care related organization; experience should include a minimum of 2 years in acute care/ care coordination.

Preferred Qualifications

  • Upon hire: Accredited Case Manager (ACM) Or
  • Upon hire: Certified Case Manager (CCM)

Requirements

Candidates must hold a Master's Degree in Social Work or a related field and possess a Washington Clinical Independent Social Worker Associate License. A minimum of 3 years of social services experience in a healthcare organization, including 2 years in acute care or care coordination, is required.
