A day in the life of a Unit Based Pharmacist at Hackensack Meridian Health includes: Validate medication reconciliation: Reconcile medications upon admission for admitted patients. Reconcile medications for patients based on criteria and patient needs prior to discharge to ensure continuity of care. Perform patient assessment and counseling: Develop individualized treatment and monitoring plans for patients. Educate patients about new medications. Counsel patients who are prescribed high-risk medications or complex medication regimens on their discharge medications during inpatient hospitalization. Facilitate discharges: Identify barriers to patient medication adherence and work with the interdisciplinary team to address these issues prior to discharge. Collaborate with the doctors to optimize cost-effective and evidence based: Review and validate all orders prior to the first dose administered. Monitor patient medication profiles daily. Participate in patient care rounds. Drive improved outcomes: Prevent 30 day readmissions, increase HCAHPS scores, and improve patient safety indicators. Other duties and/or projects as assigned. Adheres to HMH Organizational competencies and standards of behavior.
Qualifications: Education,
Required: Bachelor's Degree from an accredited college of Pharmacy. Possess strong interpersonal skills and work well at the team level, as well as independently. Preferred: Doctoral Degree in Pharmacy. A minimum of one (1) year Pharmacist experience in a hospital setting. Licenses and Certifications Required: Registered Pharmacist License. Candidates must hold a Bachelor's degree in Pharmacy and a valid Registered Pharmacist license. A minimum of one year of hospital pharmacy experience and Advanced Cardiac Life Support certification are preferred.
