Care coordinator

Experience

• Minimum one year community health experience with CHW, MA or CNA certificate or 5 years community health/outreach experience. Experience working with homeless, alcohol and drug addicted, and special needs populations preferred. Certain locations may require minimum one year of experience working as a certified medical professional in an inpatient or acute care setting. Examples of certified medical professions include CNA, MA, or EMT. Familiarity with social service resources in Sonoma County. Bilingual Spanish and English (speak, read, and write) required at some locations, to be identified by individual job postings. Experience working in a healthcare setting. Prior case management experience preferred.

• Skills: Proficiency with Microsoft Office applications including Outlook, Word, Excel and PowerPoint. Ability to prioritize projects, work relatively independently, manage multiple tasks, and meet deadlines. Strong organizational skills. Strong written and verbal communication skills. Ability to effectively present information, responds to questions, and generally communicates clearly and respectfully with participants, coworkers, other agencies and companies. Comfortable discussing sensitive health issues with clients. Ability to learn basic health care information and present that to clients. Strong interpersonal skills. Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone. Ability to work independently and as part of a team. Ability to work flexible hours. Good judgment, problem solving and decision-making skills. Ability to uphold client confidentiality with the utmost professionalism. Ability to travel to other offices and community locations which may include walking long distances across rough terrain and being outside for extended periods of time. • Requirements: While performing the duties of this job, this position is frequently required to do the following: Use standard office equipment and access, input, and retrieve information from a computer. Use computer keyboard with manual and finger dexterity and wrist-finger speed sufficient to perform repetitive actions efficiently for extended periods of time. Communicate effectively in person or via telephone in a manner which can be understood by those with whom the person is speaking, including a diverse population. Give and follow verbal and written instructions with attention to detail and accuracy. Perform complex mental functions and basic arithmetic functions; interpret complex laws, regulations, and policies; collect, interpret, and/or analyze complex data and information.

Vision

see details of objects at close range. Coordinate multiple tasks simultaneously. Reach forward, up, down, and to the side. Sit or stand for minimum periods of one hour at a time and come and go from the work area repeatedly throughout the day. Lift up to five (5) pounds.
