  • Range: $30.77 - $36.71 HourlyThe Care Coordinator will assist high risk, complex clients to access needed health services and complementary social services. The Care Coordinator will coordinate services and community support to achieve improved health outcomes for clients.

Duties and Responsibilities

Review daily referrals; meet with clients in order to obtain program consent and review client’s care plan, medical follow-up and psychosocial needs. Initiates contact and establishes a working relationship with patients for the purpose of providing assistance with attaining needed health and social services. Assist client in establishing individualized health goals, identifying potential problems/barriers; provide health education resources. Provide advocacy for client and monitor client’s progress. Coordinate logistics for health goal adherence. Assist in timely scheduling of primary care and other medical appointments to support client health. May attend appointments with clients. Assess client needs and provide information and referrals to identified resources and other services such i.e. shelter, food, clothing, and drug and alcohol treatment programs. Link individuals to application assistance for health coverage programs. Perform client benefit enrollment in areas where certified/ approved. Acts as a liaison between community –based organizations and the patients. Maintain good working relationship with community health partners by working as a liaison with other agencies and the community. Assist in coordinating and organizing resources for the patients stored on site : including but not limited to food, clothing health education materials. Document all clinical support tasks to meet grant requirements which may include data entry. Participate in quality assurance activities to monitor and improve client service quality. Reviews hospital inpatient and ER reports for program participants, and the organization. Follows all SRCH policies and procedures, follows partner agency policies as applicable. Assist in locating clients in support of outreach function. Care Coordinators who are Medical Assistants per programmatic needs, may perform vital signs, CLIA waived tests, and administer Immunizations per SRCH policies and procedures. Participate in on-going training and reviews. Other duties as assigned. QualificationsEducation: High School diploma or GED required. Driver’s license with clean driving record and auto insurance. Community Health Worker Certificate (CHW), Medical Assistant or Certified Nursing Assistant Certificate (CNA) with one year of community health experience, or a minimum of 5 years community health/outreach experience.

Requirements

Candidates must have a high school diploma or GED and a valid driver's license with a clean record. A professional certificate such as CHW, MA, or CNA is required, along with at least one year of community health experience.

Experience

  • Minimum one year community health experience with CHW, MA or CNA certificate or 5 years community health/outreach experience. Experience working with homeless, alcohol and drug addicted, and special needs populations preferred. Certain locations may require minimum one year of experience working as a certified medical professional in an inpatient or acute care setting. Examples of certified medical professions include CNA, MA, or EMT. Familiarity with social service resources in Sonoma County. Bilingual Spanish and English (speak, read, and write) required at some locations, to be identified by individual job postings. Experience working in a healthcare setting. Prior case management experience preferred.

Ability to work with people with a variety of backgrounds and educational levels both in person and on the phone. Ability to work independently and as part of a team. Ability to work flexible hours. Good judgment, problem solving and decision-making skills. Ability to uphold client confidentiality with the utmost professionalism. Ability to travel to other offices and community locations which may include walking long distances across rough terrain and being outside for extended periods of time.
