  • Purpose: The Case Manager will build relationships with patients at the RCC experiencing homelessness, and assist with basic needs assessments, housing search, access to public benefits and other resources as appropriate.

Responsibilities

Provide thorough screening for CSP-HI eligibility for all clients on a regular basis. Consistently engage with new clients as current clients end services. Assisting RCC client with physical and mental health, which can include, but shall be not limited to securing health insurance; addressing transportation needs; providing medication reminders and health education, encouraging and connecting clients with relevant support groups (e.g. Alcoholics Anonymous, Narcotics Anonymous, Gambles Anonyms, etc.), providing personal hygiene products, and coordinating income generation, housing resources, and food access. Collaborating with LCHC medical team for primary care, behavioral health and psychopharmaceutical services including addiction treatment, which include, attending weekly team meeting and daily check in for disposition planning, timely communication for weekend and overnight concerns as defined in RCC Protocols for using the LCHC medical oncall and Administrative oncall system; and consulting RCC protocols for management of the milieu. Registering RCC clients through the Homeless Management Information System (HMIS) for coordinated entry access. Partner with the Direct Care Supervisor to facilitate the intake process Coordinate and facilitate enrichment activities and low barrier psychoeducational groups for RCC clients such as mindfulness, meditation, breathing, etc . Conduct thorough housing assessments and assist all clients in completing housing applications, mitigating barriers to housing, and gathering required documentation. Assess all populations for crises needing immediate intervention and connect them to internal and/or external support. Engage in discharge planning including determining discharge location, writing a biopsychosocial, and writing a safety plan. Collaborate with existing community service providers to maximize resources and benefits available to all populations. Advocate for clients in various capacities. Utilize a Trauma Informed Care model which values autonomy, harm-reduction, etc. Ensure all individuals served are treated with dignity and respect. Ensure all services are provided in compliance with various funding sources. Complete all documentation and reports in a thorough and timely manner. Communicate well with supervisor, colleagues, and collaterals. Able to receive and implement feedback. Other duties as assigned. QualificationsQUALIFICATIONS / ELIGIBILITY REQUIREMENTS: Qualifications: Must have experience working directly with clients in the human services field. Must have working knowledge of statewide housing opportunities, public benefits and local resources. Working knowledge of the impact of trauma. Must be able to work in a fast paced environment. Must have strong organizational and time management skills. Must be flexible, able to work in various environments, and possess excellent communication skills. Proficient with Microsoft Office Suite and ability to quickly learn new software and tracking systems. Working knowledge of domestic violence, substance use and mental health conditions. Bilingual Spanish is a plus. MSW and LCSW candidates preferred. Eligibility Requirements: Must be willing to work in Lynn and have reliable transportation. Must be willing to primarily work 8am-4pm or 9am-5pm Monday through Friday with some flexibility as needed. Must have unrestricted work authorization to work in the United States. Requirements

Candidates must have experience in the human services field and a working knowledge of housing opportunities, trauma, and mental health conditions. A four-year degree is required, with a preference for MSW and LCSW candidates.
