  • Shift: DaysWe are looking for a new team member to lead our MDS Team who desires to make a difference in the long-term care arena and to positively touch lives. The Minimum Data Set (MDS) Coordinator is responsible for timely and accurate completion, coordination, recording, updating, and monitoring of all Minimum Data Set documentation required by state and other appropriate authorities.
  • Specific responsibilities include: Regulates and maintains the MDS/PPS program and completion schedule Develops and coordinates appropriate nursing care plan from MDS/RUG category Evaluates effectiveness of nursing care plans, and updates MDS/RAI process within completion guidelines Works in collaboration with the Interdisciplinary Team to assess the needs of the resident Monitors MDS and care plan documentation for all residents. Ensures documentation is present in the medical record to support MDS coding. Requirements

Candidates must hold an active and unencumbered North Carolina Professional RN license. Prior experience with MDS, Case Mix, and Electronic Health Records is required.
